SFS - Mercy Health Closed Pod Vaccine Clinic 

In partnership with Mercy Healthcare, we are offering vaccines for COVID-19 on May 17th, 2021.  This will take place at St. Francis de Sales School from 4:00 PM to 6:00 PM

 

This is an exciting development for all of us.  It is our hope that through vaccination, we will be able to greatly reduce or eliminate our need to isolate or quarantine students and staff.   

The second dose is June 7th at the same time as your first appointment. You must be able to make both appointments

This clinic is only open to SFS Students and Parents ages 12 and older.

Who can receive the COVID-19 vaccine?

 

The FDA has approved the use of the Pfizer-BioNTech COVID-19 vaccine for persons 12 years of age or older.  Since our clinic is taking place on May 17th, individuals planning on receiving the vaccine must be 12 years or older on May 17, 2021. Please note, this is a two-dose vaccine, the second dose will be June 7th at St. Francis.

 

Who can register for the vaccine at SFS?

  • Any SFS student, 12 years of age or older.

    • Parent or guardian of the above student.

 

What do I need to bring with me to my vaccination?

Please bring your medical insurance card, and a state-issued ID, or school student ID. 

 

What if I do not have medical insurance?

The cost of the vaccine without insurance is $42.  Please contact St Francis if you need assistance covering this cost. No one will be turned away for vaccination for financial reasons. 

 

Does a parent need to be present with anyone under the age of 18 to receive the vaccine?

Yes. Anyone 12-17 years of age will need to have a parent present with them to have the vaccine administered. 

Which vaccine will be administered?

Pfizer-BioNTech COVID-19, which is the only vaccine currently approved for individuals 12 years of age and older. 

 

Will I need a second dose of the vaccine?

Yes, the second dose is scheduled for June 7th. The time you scheduled for June 7th will be the same time you are scheduled for on May 17th.

 

What are my next steps to register for the vaccine?

Go to www.sfsknights.org/vaccine select the time you are looking for. Fill out your responses, and once you are registered you will get a confirmation email right away with your ticket for each person registered. Please present this ticket when you arrive. If I am sick can I still attend?

No, we ask if you aren’t feeling well, or if you are actively in Isolation or Quarantine that you do not attend. If you have had COVID in the past 90 days please check with your doctor if you should receive the vaccine.

 

What if I am not available on either date or need to cancel my appointment?

If you are not available, St Francis can work with you on finding another location to receive the vaccine. We do not currently have any make-up dates.

If you receive the first dose at SFS you must receive your second at SFS. If you have received your first dose somewhere else, you are not able to receive the second at SFS. This is due to how the vaccine is allocated to the providers.

I am unsure if I, or my son,  should receive the vaccine, or if it is right for me. Where can I get more information about the vaccine?

Day of the Vaccine Administration 

-Please bring your ticket with the QR code visible when you arrive. (Your ticket is attached to this email) There will be a ticket for each person registered.


-When arriving for your appointment please enter the school at the West Lobby doors. 


-Please do not arrive more than 15 min before your appointment. 


-If your appointment time is missed we will try to accommodate another time, but can not guarantee you can receive the vaccine if the appointment time is missed.


-Any student ages 12 - 17 must have a parent or guardian present when they receive the vaccine.

-There will be a 15 min waiting period after you receive the vaccine as part of the post-vaccine monitoring.


-Please do not attend if you are feeling sick, have been exposed to COVID, or are actively in Isolation or Quarantine.
