June 26th and 27th 6:00 to 8:00 pm at SFS. Enter via the West Lobby.

ALL ATHLETES will undergo the following testing: height, weight, vision, blood pressure,
heart/lung sounds and an orthopedic evaluation to complete the required OHSAA required Pre-
Participation Exam before sports begin.
ALL ATHLETES are encouraged to attend as the physicals last an entire year, which means you
will not have to worry about getting another physical in the middle of a sports season.

Please have the OHSAA Physical Form completed before your appointment.

When signing up for a time slot please input the athlete's name receiving the physical.

Please arrive on time for your appointment.

Parents are welcome to follow the student during this, but is not mandatory.
