What to bring the night of your registration.

Complete the application form EdChoice App.

 
Gather together a copy of the supporting documents outlined on the application which include:
 

1. A copy of the Student’s state issued Birth Certificate

2. Address Verification (please provide one of the following)

   • All pages of a utility bill showing a matching billing and service address (The address MUST be listed twice.  If you use a Toledo Edison bill, the second address is located on the payment stub).  The name and address on the bill must match the parent/guardian name and address on the EdChoice application.  Cell phone bills will NOT be accepted.

   • Mortgage Statement/Lease Agreement along with a form of Business Mail (i.e. credit card bill, bank statement, storage unit bill). The name and address on the statement must match the parent/guardian name and address on the EdChoice application.

Registration will be in the Student Achievement Center. Please enter through the Main Entrance
